Emerald Care is locally owned and lovingly operated, providing short-term and long-term in-home care and assistance for all ages.

Our team of well-trained professionals provides a variety of activities to support daily living and other services so those in need can continue living in the comfort of their own residence. Our clients include those that are getting older, are ill or disabled, individuals that are recovering from surgery, or simply those that need some extra assistance.

Emerald Care provides all types of support services, including but not limited to: personal care assistance, household care assistance, meal preparation, errand running and assignments, and many other services. If you don't see your needed service on our website, please feel free to call and ask if we can provide it.

Emerald Care also provides Private Care Management that requires a higher level of skill and health care assistance (typically involving a licensed health care professional), including these services:

• Initial client consultation and intake (complementary).
• Perform a geriatric assessment which includes a psychosocial and health assessment – a “needs and gap” assessment.
• Develop, implement, and modify a Care Plan with input from the client and/or authorized representative.
• Perform weekly or per contract on-site visits.
• Oversight and review of hired Caregivers and/or CNAs.
• Medication review, assistance, and setup.
• Consult with professionals including, but not limited to; physicians, public agencies, conservators/guardians, elder law attorneys, and family members/loved ones.
• Schedule and accompany the client to medical appointments.
• Medical bill reconciliation.
• Transition support to an assisted living or skilled nursing facilities.
